I accept most insurance plans, and am an in-network provider for many managed-care plans, including the plans listed below. If in doubt, please contact your insurance company to be sure.
Also, please be sure to obtain pre-authorization if your plan requires it. You'll find the number listed on your insurance card. Please bring your insurance card with you to your first appointment. Sessions typically last 50 minutes.
Fees: My standard fees are $150 for the first session and $125 for subsequent sessions. If you are covered by an insurance plan in which I am a participating provider, the fee may be reduced to comply with the contract fee for the plan. You are responsible for any deductible or co-pay required by your insurance, and payment is expected at the time of service unless other arrangements are made.
If you are not covered by insurance, I offer a sliding scale based on income. Please call me to discuss this.
